PARENT(S) NEED TO BRING THE FOLLOWING:
  • PROOF OF RESIDENCE IN THE LINCOLN ATTENDANCE AREA:
    (DISTRICT WILL ONLY ACCEPT A CURRENT GAS OR ELECTRIC BILL)
  • CALIFORNIA DRIVER LICENSE OR IDENTIFICATION CARD
  • COMPLETE IMMUNIZATION RECORDS

ALL STUDENTS ENTERING 7th GRADE MUST HAVE A MINIMUM OF 2 HEPATITIS B SHOTS & MMR BOOSTER SHOT.
NEW STUDENTS ENTERING FROM OUT OF STATE MUST ALSO HAVE MANTOUX TB TEST (INCLUDING TEST RESULT)

  • ORIGINAL BIRTH CERTIFICATE OR PASSPORT
  • MOST RECENT REPORT CARD/PROGRESS REPORT
  • COURT CUSTODY PAPERS (IF JOINT CUSTODY)
